Benefits of being an AHA Heart Failure Certified Center:
Certification: Less than 1% of skilled nursing facilities (SNFs) nationally have achieved this American Heart Association certification. ^1
Confidence in our care and outcomes: The hospital and patients can have confidence that we have been highly vetted by the AHA expert review process to meet their heart failure care standards. ^1
Improved care coordination: Certification improves health care coordination and communication between the hospital and our SNF, thereby enhancing the continuum of care for the patient for improved outcomes. ^1
Reduced readmissions: Heart failure patient care in our SNF impacts readmission rates, positively or negatively. AHA Heart Failure certification helps reduce unnecessary readmissions to the hospital. ^1
